Application Instructions & Supporting Documentation
Applying for an occupational therapy license in Indiana involves several steps and the submission of specific documents. Here's a detailed breakdown of what you need to do:
- Application: Submit your application through MyLicense.IN.gov along with a $100.00 application fee.
- Criminal Background Check: Ensure that your criminal background check is completed and submitted.
- Positive Response Documentation: If you answer "Yes" to any questions on the application, provide a detailed statement explaining the violation, location, date, cause number, and disposition. Include copies of court documents to support your statement. Upload these documents at the time of application or log back into your account to use the License Update option.
- Name Change Documentation: If your name differs from that on any of your documents, provide legal name change documentation, such as a marriage certificate or divorce decree. Upload these documents at the time of application or use the License Update option.
- Transcript: Submit a transcript from the school where you obtained your degree, showing the degree conferred. Graduates of foreign programs must provide an official translation if the transcript is not in English. Upload this document at the time of application or use the License Update option.
- Diploma: Provide a certified copy of your diploma or an original letter from the school indicating your degree completion and graduation date, signed by the program director or dean. Upload this document at the time of application or use the License Update option.
- Exam Score Report: Ensure that your score report is submitted directly to the Indiana Occupational Therapy Committee from NBCOT.
- Verification of State Licensure: Submit verification of licensure from every state where you hold or have held a license or certification. This form should be submitted to the Indiana Occupational Therapy Committee by the respective state. You may need to contact the state(s) beforehand to determine if they require this form.
Temporary Permit Applicants
If you are applying for a temporary permit, follow these additional steps:
- Submit your application for full licensure first, then submit your application for the temporary permit with a $50.00 application fee.
- Complete and submit the Affirmation of Supervision section found on page 3 of State Form 43826. Upload this document at the time of application or use the License Update option.
- A person with a temporary permit may only practice under the direct supervision of an Indiana certified occupational therapist responsible for the patient.
- A temporary permit expires on the earliest of the following: the issuance of a permanent license, the disapproval of the license application, or 180 days after the permit is issued.
- The committee may renew a temporary permit if the person was scheduled to take the next examination but did not take it and can show good cause for not taking the examination.
