Application Process
The application process for SLP licensure in Oregon is primarily conducted online. All other applications should be submitted through the Applicant Portal.
For those reactivating a license, direct communication with the office is required. E-transcripts are highly encouraged and can be sent to speechaud.board@state.or.us.
Requirements for SLP Licensure
- Official Transcripts: These must be sent directly from the university, particularly if the degree is from an ASHA-accredited program. Additional documentation may be required for non-accredited programs.
- ASHA CCC's or Praxis Exam: Verification from ASHA or a passing Praxis Exam Report must be submitted.
- Professional Development: Depending on the timing of your graduate program or fellowship, professional development hours may be required.
- Verification of Good Standing: Letters from all jurisdictions where you held a professional license are necessary.
