Licensure Information
To become a licensed Speech-Language Pathologist in Wisconsin, you must complete the following steps:
- Application: Complete the application and pay the applicable fees online via LicensE.
- Certificate of Professional Education (Form 1984): After you graduate, your school will submit this form directly to the Hearing and Speech Examining Board.
- American Speech-Language Hearing Association (ASHA) Certification (Form 1976): ASHA will submit this form directly to the Hearing and Speech Examining Board.
- Verification of Licensure in Other State(s) (if applicable): You must have each state board where you have ever been licensed submit letters of verification to the Wisconsin Hearing and Speech Examining Board. These letters must include your date of birth, license number, date of issuance, date of expiration, and any disciplinary actions.
Temporary Licensure
If you are commencing a post-graduate clinical fellowship, you will need to obtain a temporary license. The requirements are:
- Application: Complete the application and pay the applicable fees online via LicensE, including a $10.00 non-refundable temporary license fee.
- Certificate of Professional Education (Form 1984): After you graduate, ask your school to submit this form directly to the Hearing and Speech Examining Board.
- Temporary License Application (Form 1979): Your supervisor should submit this form directly to the Hearing and Speech Examining Board.
Late Renewal of Licensure
If your license has expired for 5 years or more, follow these steps for late renewal:
- Application: Complete the application and pay the applicable fees online via LicensE.
- Completion of Continuing Education: Provide proof of completed continuing education.
- Verification of Licenses: Submit verification of all licenses (active or inactive) from other states.
Additional Requirements
Your application is not complete until all of the following documents have been received:
- Complete application and pay applicable fees online via LicensE
- Letters from all State Boards where licensed, active and inactive
- Certificate of Professional Education (Form 1984)
- Verification of Certification from the American Speech-Language Hearing Association (Form 1976)
- Proof of Continuing Education (Late Renewal applicants only - License expired more than 5 years)
- Convictions and Pending Charges (Form 2252), if applicable
- Malpractice Suits or Claims (Form 2829) and copies of malpractice suit, court documents with allegations and settlement, if applicable
- Certified copy of marriage certificate, divorce decree, etc., if the name on all credentials is not the same
